The report continues, “Following the investigation, PHMG undertook a comprehensive review of all the files potentially impacted to assess the information contained therein and to whom it related, and to locate address information for all affected individuals. On September 4, 2025, PHMG finalized this review and determined that information related to you could be affected.”
Palomar Health could have prevented the breach with better cybersecurity. plaintiffs have argued. PHMG hasn’t admitted to any wrongdoing but agreed to the settlement to avoid further court action.
Under the settlement, class members who suffered documented losses can claim up to $5,000.
Eligible documents to support claims include bank statements, receipts or other financial paperwork, The U.S. Sun noted on Friday, September 18. The amounts are subject to change based on the number of successful claims. All class members can receive two years of free credit monitoring.
The deadline to submit a claim form is October 22, and payments will be distributed following a November 6 final approval hearing. Claim forms can be submitted here.
