Carson Whisenhunt Scheduled For Tommy John Surgery Next Week

The Giants already made it known last month that young lefty Carson Whisenhunt had been diagnosed with ligament damage in his left elbow and would require surgery of some kind. Whisenhunt was headed for another opinion to determine whether he’d need a full UCL reconstruction (i.e. Tommy John surgery) or an internal brace procedure. It seems that additional opinion recommended he undergo the more invasive of the two options; the Giants’ latest medical report indicates that Whisenhunt will have Tommy John surgery on Sept. 16 (via MLB.com’s Maria Guardado).

An internal brace procedure in late August might have given Whisenhunt an outside chance at returning for the final few weeks of the 2027 season. However, a mid-September Tommy John procedure all but ensures that he’ll be sidelined all the way into the 2028 season. Typical recovery for Tommy John surgery these days is closer to 14 months. Internal brace surgery tends to come with a slightly shorter timetable.

    The No. 66 overall pick in the 2022 draft, Whisenhunt has consistently ranked among the Giants’ top prospects and at one point even cracked top-100 lists at MLB.com and Baseball Prospectus. He’s been hit hard in the majors, however, recording a 5.90 ERA in 13 major league starts and 58 innings dating back to 2025. He pitched well in Triple-A this season and was solid through his first two big league starts before being lit up for 21 runs in his next 24 frames (with a 16.9% strikeout rate and 12.7% walk rate along the way). Whisenhunt pitched 2 2/3 shutout innings in his final appearance before exiting that start due to elbow discomfort.

    Since the injury occurred in the majors, Whisenhunt will receive major league service time and salary while he rehabs. He’ll finish this season about a bit less than a month shy of a full year of major league service. By the time he’s back in ’28, he’ll have nearly two full years of service time. The rest of the group lacks certainty. Veteran Adrian Houser is signed for $11MM next year but hasn’t been able to hold a rotation spot this year and was recently shelved due to a forearm strain. Trevor McDonald underwent his own Tommy John procedure last month, as did Hayden Birdsong back in March. Young arms like Blade Tidwell, Matt Wilkinson and Cesar Perdomo are getting looks down the stretch but haven’t proven much yet. The Giants are trying former Rockies and Braves reliever Anthony Molina as a starter, too.

    The Giants entered the 2025-26 offseason in clear need of notable rotation upgrades but instead opted to sign back-end veterans Houser and Tyler Mahle — the latter of whom has since been traded to Atlanta. With Robbie Ray now in San Diego following a deadline trade, the need this winter is even more acute. The Giants did add some interesting pitching prospects ahead of the deadline, acquiring not only Molina but Ramon Marquez (for Luis Arraez), Henry Lalane (for Heliot Ramos) and Miguel Mendez (for Ray) in respective trades with the Phillies, Yankees and Friars. They’ll still need to bring in at least two steady arms if they have any designs on returning to contention next season, though.
