Triston Casas Undergoes Wrist Surgery

The Red Sox announced that first baseman Triston Casas underwent left wrist arthroscopy today. They did not provide an expected recovery timeline. He is already on the 60-day injured list.

While the next steps are unclear, it adds yet another hurdle for Casas in his gruelling injury odyssey. It’s now been well over a year since Casas played in a game. On May 2 of last year, he was trying to beat out a ground ball when he landed awkwardly at first base and collapsed with an obvious injury. He had to be stretchered off the field and later required season-ending surgery. He still wasn’t recovered by the start of this season. He then suffered a strained left intercostal muscle shortly after Opening Day. He was transferred to the 60-day injured list in April.

On June 21, Tim Healey of The Boston Globe reported that Casas had gotten healthy enough to hit off a tee and do some soft toss. Ideally, he would have continued ramping up his baseball activities before heading out on a rehab assignment. But he was shut down about a month ago, per Healey, due to his left wrist. It seems that Casas and the club explored some options before determining surgery was necessary.

It may be hard to remember now but Casas once looked like a long-term building block for the Red Sox. He stepped to the plate 840 times from 2022 to 2024 and hit 42 home runs in that time. His 26.9% strikeout rate was a bit high but he counteracted that with an excellent 14.2% walk rate. He had a .250/.357/.473 line and 127 wRC+ for those three seasons.

But he got out to a slow start in 2025 and has been battling multiple injuries since then. Coming into this season, the Sox didn’t want to count on him, so they acquired Willson Contreras from the Cardinals. Contreras is signed through 2027 with a club option for 2028, so Casas doesn’t have a great path to retaking his former position. The designated hitter spot is currently occupied by Masataka Yoshida, who is signed through next year.

Even if Casas can get healthy by next year’s spring training, it’s unclear where he stands with the Red Sox. They could certainly keep him around. He is under arbitration control through 2028. He is making $1.61MM this year. When an arb-eligible player misses an entire season, they usually make the same salary in the following season, if they are tendered a contract. That salary figure isn’t too far above this year’s $780K league minimum. It’s possible the next collective bargaining agreement pushes the minimum up and narrows the gap even further.

Tendering a contract to a guy without a path to playing time might seem a bit odd but Casas still has options. After missing almost two entire seasons, spending some time getting reps in Triple-A might be good for him. The Sox would potentially have a good backup option for an injury to Contreras or Yoshida. As a depth option, he would be making more than most minor leaguers but the cost would still be a drop in the bucket for a major league team, especially a fairly big spender like the Sox.

They could also explore other avenues. Perhaps there’s a rebuilding club out there who has some runway to let Casas play in the big leagues. That would be selling low for the Sox but might be preferable to just having him in Triple-A. If the injuries are enough to scare off all clubs, then perhaps he ends up non-tendered and on the open market, where he could be an interesting buy-low opportunity. Those questions will be answered in time. For now, Casas still has to focus on his health.
