Chalobah was left unable to continue in the 57th minute after receiving treatment.
After a couple of minutes, he was replaced by 19-year-old Josh Acheampong.
And it leaves the Blues in something of a defensive crisis.
Levi Colwill was hit by a season-long ACL injury earlier this week after he underwent successful surgery before reporting back to Cobham.
Maresca had called signing a replacement a “priority” following Chelsea’s 2-0 win over Bayer Leverkusen on Friday.
And with Wesley Fofana also sidelined, it leaves the recently returned Benoit Badiashile – who was not named in the squad to face Milan – and inexperienced teen Acheampong as the only realistic options to play in that role when the club face Crystal Palace in their Premier League opener next week.
