(KRON) -- None of the eight people who died in a boating accident during a freak storm on Lake Tahoe last month was wearing a life preserver, according to a new report from the National Transportation and Safety Board. One survivor put on a personal flotation device (PFD) and distributed them to everyone else aboard the boat.
However, no one else immediately donned one. Shortly after that, the boat, a 28-foot recreational vessel named "Over the Moon," rolled over to starboard and the 10 passengers went into the lake. According to the report, a surviving passenger recalled that it began to snow after the boat rolled over, dumping everyone into the water.
No distress calls were made from the boat during the June 21 incident, the report states. On the shore, however, a group of hikers saw the boat capsize after a large wave washed over it at around 2:57 p.m.

Multiple agencies responded to the scene including paramedics and a sheriff's marine unit. The marine unit reported 8-foot waves in the area.
Eight people were recovered in the tragedy, including two survivors with flotation devices. Six of those recovered, including the boat's operator, were pronounced dead at the scene, despite CPR efforts, the report states.

Of the two survivors, Amy Friduss and her mother Julie Lindsay, one was wearing a life preserver, and another was clinging to one. Both survivors were transported to the hospital for treatment.
The two remaining passengers were recovered from the bottom of the lake the following day. Four life vests and one life ring from the vessel were recovered from the accident site.
