TIME is running out for eligible Americans to claim up to $10,000 from a sizeable data breach settlement.
Just one document is needed for those affected to claim their money.

The settlement benefits individuals whose data may have been compromised in a data breach on April 6, 2023 and who received notification about this.
Plaintiffs in a class action lawsuit claim the health care company failed to put adequate safeguards in place to prevent the breach.
They claim that they suffered damages including the risk of identity theft and fraud.
Enzo Biochem has not admitted any wrongdoing.
However, it has agreed to pay out $7.5 million in a class action settlement to resolve the claims.
Class members can get up to $10,000 for losses related to the data breach, under the terms of the settlement.
This might include unreimbursed fraudulent charges, identity theft expenses and credit monitoring costs.
Any who did not experience documented damages can opt for a cash fund payment instead.
All class members will be eligible for two years of free credit monitoring and insurance services.
Enzo has also agreed to improve its data security.
These promised improvements include multi-factor authentication and encryption of consumer data.
A final approval hearing for the settlement was set for June 10.
The deadline for claims is June 23 – meaning time is running out for those affected to file a claim.
Claimants just need to provide documentation of unreimbursed losses, such as bank and credit card statements.
Elsewhere, payments are part of a $20 million class action lawsuit brought against Aetna, Brightline, Community Health, and others.
This breach occurred in January 2023.
Claimants can receive money if they fill out a claim form by the August 29 deadline.
