The teenager's mental health is suffering, as a result of blackmail attempts from vile groomer Megan Walsh (Beth Nixon) and her victim Will Driscoll (Lucas Hodgson-Wale).
In the aftermath, he became hooked on ADHD medication to manage his stress levels, and collapsed in the street.
He finally addressed his fears, but that didn't end his turmoil. He began to envision Will threatening to kill him, and in more recent weeks, has seen multiple versions of his best pal Roy Cropper (David Neilson).
Walking down the street, he zoned out following a conversation with Chesney Winter-Brown (Sam Aston), and heard Roy's voice echoing through his head. He felt compelled to head to the Rovers Return to meet with Will, who was playing a shooting game on his console.
'Roy' warned that Will was going to try and kill him too, and he darted out to the safety of the cafe.
The trio of parents were alerted to the unfolding situation and cried out for Sam to accept help. Unfortunately, he listened to the imaginary Roy's orders, and threw a chair across the room towards the real Roy.
By the time the group managed to gain access to the cafe, Sam had run off again and Roy was injured on the floor.
When Sam caught sight of his dad approaching, he tried to escape and was wrestled to the ground. Carla explained that the situation reminded her of her own psychosis, and that he should be taken to hospital.
An exhausted Sam fell asleep in the car, and when he awoke, was horrified to see that they'd arrived at Weatherfield General.
