Showrunner Allen MacDonald played coy about whether fans should be “worried” about Lenox (Sarah Ramos) surviving the season, exclusively telling Us Weekly the team wanted to “keep it a complete mystery” after her dramatic cliffhanger.

“I will say that we do a time jump. We don’t just pick right up where we left off. We jump ahead two days,” MacDonald confirmed ahead of the show’s winter return on Wednesday, January 7.

During the Chicago Med fall finale in November 2025, Lenox trusted her gut and went to former patient Faye’s home during a massive storm to find her beaten and bleeding in her basement. While Lenox tried to help Faye (Olivia Nikkanen), her counterpart Dr. Mitch Ripley (Luke Mitchell) did his best to keep Faye’s abusive partner, Devin (Jack Falahee), at the hospital after he came in with an alleged hockey injury.

According to MacDonald, the domestic violence story line is “nothing new,” but an issue that often goes “unchecked,” which is why Lenox is so invested in saving her patient — even at the risk of her own safety.

“[It’s] something that just infuriates her that he would do this to her. It also infuriates [Lenox] that when we first met them, she tried to give Faye away out, and Faye didn’t take it,” MacDonald explained of Lenox’s state of mind. “That’s part of the infuriating cycle of that abuse. That created the impatience and kind of raw fury that I think Lenox felt in the fall finale and fueled her to find her way to their house.”

Although the showrunner wouldn’t reveal whether Lenox made it out of the house safely, he teased fans have a lot more to keep up with when the show returns — including the connection between pregnant Dr. Hannah Archer (Jessy Schram) and her so-called platonic baby daddy, Dr. Dean Archer (Steven Weber).

“They are two friends having a baby. That is sort of the agreement that they made in the first scene of the season, and I think they’re both trying to stick to that,” MacDonald told Us of the friends and coworkers. “But I do think they both have feelings they can’t quite control or describe or don’t always admit they have.”

He noted that the unresolved feelings between Hannah and Archer will cause “some complications coming up in the future” and they will be forced to “confront” those issues.

MacDonald teased that Dr. Jennifer Kingston (Merrin Dungey) showing “some interest” in Archer during the fall finale will add to the tension between him and Hannah as she carries their baby.

“That’s going to move forward a little bit and bring up some questions for Hannah,” the showrunner confirmed.
